3. SUBMIT RENTAL APPLICATION

To expedite the rental process, you may also submit a RENTAL APPLICATION along with a note detailing your rental needs.

The RENTAL APPLICATION should be completed, signed, and submitted by dropping off at our office or fax to: 808-822-5218.

You’ll need the free Adobe Acrobat Reader to view and print the PDF form. If you don’t already have this software, you can download it at the ADOBE site.
